Your residence document has been stolen or lost. Foreign national's document is another name for residence document. This also includes a Foreign Nationals Identity Document (Type W and W2). You first go to the police to report the theft/loss. Clearly indicate that you are reporting the theft or loss of a foreign national's document issued by the IND. The police will give you a certified copy of the official report. When you send the application to replace the residence document, you enclose a copy of the official report.
Has your residence document been stolen or lost during a stay abroad? You then, also, first go to the police to report the theft/loss. The report has to be drawn up in Dutch, English, French or German. Is this not the case? The report then has to be translated by a translator certified by a Dutch court.

Did you report the theft/loss to the police? This makes the residence document permanently invalid. You then always have to apply for replacement. Your residence document is damaged in such a way that you can no longer use it as a valid identity document. You enclose a copy of the front and back of the damaged residence document with the application to replace the residence document. You have to hand in the damaged document when you collect the new residence document.
Your residence document has the wrong information on it. For example, a wrong or old name, date of birth, place of birth or nationality.
The IND uses the information from the Municipal Personal Records Database (BRP). Therefore first contact your municipality. They will check if your personal details are entered correctly in the BRP. If your personal details in the BRP are not correct, then these have to be changed first. Only then can you ask the IND to replace your residence document.
You have built-up employment rights and you are now allowed to work without a TWV. You can apply for a new residence document with the following employment rights: Work allowed free from restrictions, TWV not required. You do not have to pay for this application.
Is your residence permit valid for less than 1 month? Then you can apply for an extension. You can use application form number 7016. Write on the form that you wish to have the labour market notification "arbeid vrij toegestaan" (work freely permitted) on your new residence permit."

How do I complete an online application in case of damage or incorrect personal details?
A TWV is a work permit. The employer applies for a TWV with UWV (Netherlands Employees Insurance Agency). UWV examines among other things if there is no employee with the Dutch, EU/EEA or Swiss nationality that is qualified for the job.
Nowadays you can arrange many things yourself on government websites. With DigiD you log in to your personal account on government websites. Your DigiD consists of a username and password that you both choose yourself. Don't have a DigiD yet? Request this on www.digid.nl.
To apply for a residence permit online you need to have a DigiD with SMS code. After logging in with your DigiD you will receive an SMS with a code. You will have to fill out this code as well to get access to the online application forms.
Do you have a DigiD but no SMS code verification? Please go to Mijn DigiD and log in. Choose 'Extra sms-controle aanvragen' and enter your mobile phone number. Within 5 days you receive a letter with an activation code. Activate the sms control functionality on the website of DigiD with this activation code.
