Your residence document has been stolen or lost. You first go to the police to report the theft/loss. To report the theft/loss you need your v-number. You can find the v-number on letters from the IND.
The police will give you a certified copy of the official report. When you send the application to replace the residence document, you enclose a copy of the official report.
Has your residence document been stolen or lost during a stay abroad? You then, also, first go to the police to report the theft/loss. The report has to be drawn up in Dutch, English, French or German. Is this not the case? The report then has to be translated by a translator certified by a Dutch court.

Without having a residence document, you may need a visa to travel back to the Netherlands. Please contact the Dutch representation in the country you are staying.
Did you report the theft/loss to the police? This makes the residence document permanently invalid. You then always have to apply for replacement. Even when you find the residence document again, you can no longer use it. You must hand in this residence document to the IND. You can do this at an IND Desk. For example, when you collect the replacement residence document. Or, you can send the document, that you found again, to the following address:

Your residence document is damaged in such a way that you can no longer use it as a valid identity document. You enclose a copy of the front and back of the damaged residence document with the application to replace the residence document. You have to hand in the damaged document when you collect the new residence document.
Your residence document has the wrong information on it. For example, a wrong or old name, date of birth, place of birth or nationality.
The IND uses the information from the Municipal Personal Records Database (BRP). Therefore first contact your municipality. They will check if your personal details are entered correctly in the BRP. If your personal details in the BRP are not correct, then these have to be changed first. Only then can you ask the IND to replace your residence document.
You have built-up employment rights and you are now allowed to work without a TWV. You can apply for a new residence document with the following employment rights: Work allowed free from restrictions, TWV not required.
The application form Application Replacement, renewal or first application for a foreign nationals document (6013) does not have an option to indicate you want to change the employment rights. In section 3 'Statement of circumstances' you can indicate that you wish to have the employment rights on your residence document changed.
You apply for the residence permit to the IND yourself. Use the application form. You can find the form at the bottom of this page. You send the application form together with the required documents to the IND by post.
You pay for the application upon collecting the residence document.
You do not have a passport and you cannot get one? Maybe you can get a Foreign National's Identity Document.
After your application has been received, the IND sends an confirmation letter. The confirmation letter states the date the IND has received your application. It also states the period within which the IND makes a decision.
You can now go to the IND desk to give your biometric information. You do not have to make an appointment for this.
The IND will then check to see if your application is complete and that the fees have been paid. Are documents missing, or are documents not correct? In that case, the IND will send a letter: a request to rectify the omission. This letter lists the documents that you still needs to be sent. Send the requested documents before the mentioned date to the IND.
The IND checks whether you meet all the conditions. The IND tries to make a decision within 8 weeks.
If you do not meet the conditions,you will not get a new residence permit. This decision is given in the formal decision. The IND will send the decision by post. You can apply for a review of this decision.
If you meet all the conditions,you will get a new residence permit. The IND will send you a letter indicating when you can collect the residence permit.
Use this form to replace your temporary or permanent residence permit when damaged, lost, stolen or when your personal details changed. You can also use this form to replace your document verification to EU community law or long term resident EU.
The application form lists the documents that you have to enclose with the application.
You have to have official foreign documents legalised and translated into Dutch, English, French or German.

Do you have a permanent residence permit or long-term resident EC? Your application costs € 159. Children under 18 pay € 51.
Do you have a residence permit to stay with a family member or continued residence after stay with a family member? Your application costs € 237. Children under 18 that stay with a family member pay € 51.
Do you have a temporary or permanent asylum residence permit? Your application costs € 159. Children under 18 with an asylum residence permit pay € 51.
Do you have a document document EU/EEA citizen of the Union? Your application costs € 51. Children under 18 that have a document EU/EEA citizen of the Union pay € 29.
Are a you Turkish citizen or a family member of a Turkish citizen? Your application costs € 51. Turkish nationals under 18 or family members under 18 of a Turkish national pay € 29.
The IND aims to have the residence document ready for you within 2 weeks after the positive decision. You receive a letter when you can collect the residence document at the IND desk. You do not have to make an appointment to collect your residence permit.
Are you younger than 18 years? You can then only collect your residence document together with your parent(s) or legal representative(s).
You pay the costs for the application at the IND Desk. You also hand in your old residence document (except when it was stolen or lost).
Check to see if the information on the residence permit is correct while you are still at the IND desk. If something is incorrect, inform the IND staff member at once.
The replacement residence document has the same end date as the original residence document.
Was the original residence document valid for less than 3 months? You will not get a new residence document, but a formal decision. This letter states your right of residence and the end date of your right of residence in the Netherlands.
The back of your residence document has the same employment rights as were listed on your original document. This is not the case when you asked for a replacement to have the employment rights changed.
A TWV is a work permit. The employer applies for a TWV with UWV (Netherlands Employees Insurance Agency). UWV examines among other things if there is no employee with the Dutch, EU/EEA or Swiss nationality that is qualified for the job.
